Demo Cedar360 Church. This goes to a real person — a pastor at your campus — who will call you, talk it through, and confirm the date. You’ll get a reference and a private link to see where it stands.

I have accepted Jesus Christ as my LORD and Saviour, and I am now living a holy life, and seeking to be His witness. Thus, in obedience to Christ's command, and in following His example (Matthew 3:15, 28:19; Mark 16:15,16), I do hereby request the Christian Rite of Water Baptism voluntarily and of my own free will and personal conviction. I further declare that no force, pressure, fraudulent means, inducement, gift or benefit has been given to me or mine, and certify the same by my signature below.
